We have made it possible to make burial arrangements by email or fax. The forms below are in .pdf format; if you don't have a .pdf reader installed on your computer you can download Adobe Reader by clicking here.

You can open, fill in, print and sign the forms, then fax to the number on the cover sheet included in the documents.  You can also scan and email the forms to info@bayareacremation.com.
