We have made it possible to make cremation arrangements by email or fax. The forms below are in .pdf format; if you don't have a .pdf reader installed on your computer you can download Adobe Reader by clicking here.

 Click on Arrangement forms and begin.

You can open, fill in, print and sign the forms, then fax to the number on the cover sheet included in the documents.  You can also scan and email the forms to info@bayareacremation.com  After speaking with you, we will then submit the Statement to you depending on the services you select.  If you have any questions about the information requested, please don't hesitate to call us at 800-916-4888.
